Impact and Programs
Accomplishments
In 2022, Light the Legacy expanded outreach to our Somali, LGBTQ+ Veteran and rural community members, hosted a unique event with the Theater of Public Policy for over 100 participants, continued developing our virtual training modules, put on 82 training events, assisted over 600 community members in completing their advance healthcare directives, and reached over 1,250 additional community members through in-person events. The biggest news of our year was our purchase of the Honoring Choices trademark. This means that in addition to the already important community work being done by Light the Legacy, our organization is taking on an increasingly critical statewide and national role. We are now the statewide resource for issues of advance care planning and are the convener of a national network of 11 states who use the Honoring Choices name.
Current Goals
Honoring Choices primary goal is to increase the percentage of Community Members who have engaged in Advance Care Planning and completed a Health Care Directive. This will be accomplished by partnering with local agencies and health care Providers to educate the community about the importance of Advance Care Planning for all age groups over 18 years of age and well as training Healthcare providers. We will identify and train community volunteers and health system staff in advance care planning and identify underserved populations for targeted interventions regarding advance care planning. In conjunction with doing Honoring Choices regular program activities, Light the Legacy has partnered with CentraCare Health to continue its work in the local communities, but also to focus on the Health Care Systems within CentraCare Health. Honoring Choices will be training Advance Care Planning (ACP) Navigators through a 5-hour virtual training. A new ACP LMS has been created with roll out scheduled in the fall of 2023. A recorded ACP video has been created to assist patients in the hospital and show to patients at the CentraCare Clinics and on their landing page on their website. A revision project for the current Honoring Choices health care directive is underway with the completion of a new and revised health care directive by the end of 2023. The last revision was in 2014.
